IU Forms
Agreement and Release
This form details the obligations of both the program participant and Indiana University Office of Overseas Study. Before participating on an IU program, students (and the parents of students considered dependents) will be required to read and sign the IU Overseas Study Agreement and Release.
Recommendation Forms
All programs require recommendations/references be submitted in support of applications. Forms and formats vary. Students can access forms through the IU online application system.
Interview Appointments for Confirmed Applicants
Should your application be accepted for consideration and an interview be required, you will receive instructions by e-mail to sign up for an appointment time. Do not attempt to sign up for an interview until receiving notification!
Post-Acceptance Advising
After you have been accepted to a program, you will need to complete the forms below and meet with your major advisor(s) and a study abroad advisor. Overseas Study will contact you by email with instructions for post-acceptance advising. At that time, you will need to access the Instruction Sheet and Advising Plan for your home campus. Please save the Advising Plan document according to the instructions provided.
